Root Canal/Endodontics:

Endodontics is the area of dentistry deals with diagnosis, prevention and treatment of the soft tissue inside the tooth. When your tooth gets injured through trauma or diseased through decay or gum infection, in the past these teeth were extracted but now with endodontic treatment, these teeth can be saved.

Tooth has three layers. Outer most layer that you see is enamel, second one is called dentine and the inner most is called pulp. Pulp has nerves and blood vessels that keeps the tooth moist and alive. Enamel is a hard layer that protects the tooth. Dentin is the middle layer, it is porous and contains nerve endings. When tooth gets decayed or gets injured, bacteria can enter into dentine and pulp. If it is not treated on time, it can set infection and that infection can spread to the tip of the root, often resulting in the swelling and pain. If not treated on time, it can spread to surrounding tissue and can be life threatening.

Endodontic treatment or root canal can save such an infected tooth, if treated on time. We numb the tooth with anesthetic, remove diseased tooth structure including diseased pulp. Pulp chamber and canals are then cleaned, irrigated , filled and sealed to prevent recontamination of the root canal. Root Canal Therapy is usually a painless procedure. With new engine driven files and new filling materials, it is lot quicker and painless. At this time, we place temporary filling.

Sometimes, if tooth is too far broken, you may need to get metal post placed in one of the canal to support the filling Otherwise, we seal the tooth with permanent filling to prevent bacteria from re-entering the canal.

Once the nerve and blood supply is gone from the tooth, the tooth dries out and is likely to fracture. So you need to go back and get crown done on that root canal tooth to restore its structure, function and appearance.

One may ask why would you do root canal, post-core and crown in stead of just removing the tooth?  There is nothing like your own tooth. When tooth is removed and not replaced, other teeth can tilt in that space and opposing arch teeth can start extruding out of their socket, too, giving cavities and gum problems to neighboring teeth and ultimately you can loose them, too. If you replace them with fixed bridge, it will require to cut adjoining two teeth to take a support  and those teeth now in stead of just taking load for themselves, have to bear extra load for a missing tooth and eventually that bridge can fail or you may need to get that bridge replaced every couple of years as cement can wear out! Bridge is not cheaper either. So you loose diseased tooth, spend money for its extraction and then spend money for its replacement with fixed bridge which itself is expensive, and then calculate additional expense for replacement down the road. If you decide to replace it with the implant, we have to make sure that you have enough bone, otherwise, you may need to get bone grafting- additional procedure and money and then implant placement which is also expensive. You also have to make sure that implant integrates with your bone, unlike your own tooth. My personal advise, I would not let anyone pull my tooth unless it is absolutely necessary!
